Esther, it sounds like you've looked at a lot of the right things. I agree
with your suggestion of getting thyroid checked, and would also add to that
prolactin, testosterone and cortisol. With prolactin, try to get a baseline
at least-- drawn when there has been no stimulation for a couple of hours.  

The minimal breast changes with flaccidness is troubling, and I've also seen
this with otherwise normal looking breasts (if non-pregnant, non-lactating).
It defies logic if the health history and reproductive histories are clean!


In the meantime, the domperidone sounds great. I personally would also
consider goat's rue, given the questionable mammary growth; it could be used
alongside the domperidone. Second choice would be one of the commercial
combination tinctures that includes goat's rue (there are at least three
that I know of). Then, mom needs to be patient and give her body time to
recover and to respond to stimulation and the galactogogues. Hopefully the
lab panel will turn something up!
