I am in New York State, but I have had several local pharmacies mix the components of APNO for moms. It does not have to be a compounding pharmacy because it is just 3 medications (that are readily available) that are mixed together.
Just get the MD to write the prescription listing the medications, strength, and amount of each to include. Direct the pharmacy, in the prescription, to mix them together, and label, "apply to nipples sparingly after breastfeeding".
Shouldn't be a problem to get if filled, and shouldn't be expensive.
