I think the simplest way to start is by asking, "Will you be breastfeeding?"
It sets up breastfeeding as the assumed choice.  Theoretically it is easier
for the mom to say "Yes" than "No".  In most cases her response, if not
planning to nurse, will give a clue where to go from there.  If she says she
can't because of....., then if what she presents doesn't preclude nursing,
the nurse can proceed to offer more correct information.  If it was due to a
bad experience previously, she can be reassured that there is help available
and more knowledge now.  Even her tone of voice in how she says "No" can
give a clue as to how to proceed.
