This was a common practice at my inner city hospital in St. Paul until
the early 80s. What stopped the practice cold turkey was an article
about a baby having a choking incident in another state with this
practice. To my knowledge, the best reason to avoid rigging up a
pacifier with a bottle nipple and cotton ball is the choking hazard. For
this reason I would also avoid a bottle nipple on the breast as a nipple
shield.
