Here is a sample of our phone call log sheet. It is made in word table
program. Picture a grid.

We can get 5-6 calls per page--no time to write, in depth, what advice we
gvie--if something is complicated we make an appointment for an outpatient
consult. We can easily get 10-20 calls per day. The person covering outpatient
consults during the week takes care of telephone advice. On weekends the
consultants covering the hospital also field advice calls. We save these sheets in
a binder and after about a year we shred them. I guess any advice that has
not come back to haunt us after a year we don't worry about. It is not exactly
a shadow chart and nobody sees these sheets except us so I think we're within
HIPPA guidelines, but who knows.
