I know the  question should be open-ended, not "breast or bottle", but I'm
wondering if  anyone out there has scripted it for their staff or if any of
you have your  own successful ways of addressing the issue that you would be
willing to  share.

How about something like, "And as soon as possible after the baby is born,
one of us will help you put the baby to the breast.."  Period.  Let  the mom do
with it what she will.  Why ask?  She always has the option  of saying, "no,
I've planned on bottle feeding this baby."  Then the nurse  can say, "Oh.  OK.
 We'll make sure we have the pump available so you  can start pumping right
away so the baby can get her milk as soon as  possible."  But leave it in her
corner.  Wouldn't it be a neat idea if  we all made an assumption that every
mom was going to breastfeed?  And that  if she has chosen to bottle feed, that
we *assume* she is planning to give her  baby breastmilk?
