Hello, Rachel.
You say "Mom wonders why I say yes and he [ped.] says no?"
If you read directly from the Hale book to the mother, *you* have not
said yes, *Dr. Hale* has said yes. And really, you should read directly, as
well as tell her what book you are quoting from, rather than interpret what the
book says. Better liability protection for you.
That said, you can point out that Dr. Hale's information is *not*
taken from the manufacturer's insert, which tends to be extremely conservative to
avoid liability issues (CYA, if you catch my drift). Dr. Hale's information
is gathered from research studies, case studies, personal communications, and
using the chemical nature of the drug as to how it metabolizes, etc. You could
suggest that the mother ask her pediatrician where he gets his information.
Let the mom know that the American Medical Association has told doctors that
the PDR (PHYSICIANS' DESK REFERENCE) is *not* the source to refer to about
safety questions for medications during breastfeeding. Then, if her doctor says
he looked in the PDR, the mother knows how much weight to give his answer.
