Finally - here's the one we used. It was weeded down to 6 questions on one
page per mandate of Mgmt. The weeding down, and some wording editing, was
with Mgmt's input. We had others we thought were important, but were
overruled. There is no copyright, and since I helped write these, you can
use them, edit them, or whatever. We looked at a number of published BFing
Q&As, and focused on topics where we had observed a variety of opinions &
knowledge levels among staff, and that variety affected the instructions
they gave moms. This also led to lots of discussion among the staff as they
collaborated on their answers. That was a major objective.

Breastfeeding Needs Assessment Questionnaire

1. The best time to establish breastfeeding after birth is:
        A. Within 30 minutes
        B. Within 2 hours
        C. Within 24 hours
        D. Before discharge
2. Mother says she wants to feed both "breast and bottle". Your best
response is:
        A. Teach mom about exclusive breastfeeding
        B. Teach mom both breastfeeding and correct formula feeding
        C. Routinely feed baby formula at feeding time whenever baby is in the NBN.
        D. Make sure she always has a bottle of formula in the bassinet.
3. Mother says her nipples hurt. Your best response is:
        A. Tell her that nipple pain is normal and will pass with time
        B. Give her a nipple cream or breast shells
        C. Teach her how to latch baby onto nipple
        D. Teach her to limit feeds to avoid soreness
4. Mother says she has "no milk". Your best response is:
        A. Offer formula because baby must eat
        B. Tell her it's okay to wait until her milk comes in
        C. Tell her to try to breatfeed, then top off with formula
        D. Tell her there is milk. The amount is low so baby can learn
5. Mother says baby wants to feed "all the time". Your best response is:
        A. Don't allow that or you will get sore nipples
        B. That's proof you don't have enough milk. Offer a bottle of formula
        C. Assess baby's latch onto the breast to be sure there is good milk
transfer
        D. Tell her baby is learning to breastfeed. He will get more in less time
with practice
6. How many voids and stools would indicate a totally breastfed 10 day old
is receiving sufficient breast milk?
        A. Seven (7) clear voids and a green stool every other day
        B. Four (4) yellow voids and two (2) yellow stools daily
        C. Six (6) clear voids and three (3) yellow stools daily
        D. Three (3) yellow voids and two (2) brown stools daily

That's it. It's not perfect. It was done by a committee, half of them with
no BFing training and they outranked us.
