was called to see a 6 m.o. boy yesterday because of zero weight gain in
the past 2 mos.
Birth weight 7lb ~6oz. Weight at 4 mos: 14lb 13oz. Weight at 6 mos: same.
Baby did grow 1/2" since 4 mo checkup. Docs not happy with the growth
either. Mom said head circumference is increasing
Mom says in the last month he has been nursing "all the time". He used to
sleep thru the night (8-10 hrs) but now wakes up about 2x night to nurse or
eat.
I also suggested that she
have him stay on one breast for several "pit stop" feeds in a row, perhaps
for 3-4 hours on the clock before switching to the other breast, in hopes
of upping the cream / calorie intake.<<
Since we know that a more empty breast has higher fat content ALREADY,
decreasing this baby's intake (volume) by staying with 1 breast for 3 hrs will give
this baby even less calories!?
I would be sure that she uses both breasts per feed, 2 positions each breast
w/ compression. Following up with pumping might allow her to accrue some milk
(high in fat ) over the course of the day which she can offer at night or
whenever.
Of course there is a valid argument that since this baby looks great AND
doubled his BW prematurely (according to the BIBLE),'he doesn"T need intervention
but that is another story.
Following up on a previously posted similar case;
HUNGRY 4 mth old with consistent wt gain of only 1.2 oz day. Mom tried 2
weeks of fenugreek along with above regime. Described improved letdowns, fuller
breasts, more satiated baby at 1 wk BUT 2 wk wt check revealed same substandard
pattern of gain. Pedi suggested nutritionist but Mom elected to try
domperidone with above, w/i 48hr described breasts "twice the size", baby much happier
between feeds etc. 1 wk check revealed 10 oz wt gain! Yahoo!
