Has the baby's intake been monitored with pre and post-feed weighing ? What
is the real milk transfer this baby succeeds in achieving ?
If we follow Hartmann and team's research, if the baby does not empty the
breast sufficiently (because of poor milk transfer) for whatever reason, the
milk production will down regulate. Moreover, the baby will be getting
low-fat foremilk constantetly and thus never get to the fatter hindmillk. No
matter how much domperidone this mother takes, if the breasts are not
sufficiently emptied, the milk production will down regulate. This may
explain the slow weight gain for this baby.
