You may want to look more specifically for signs of insulin resistance vs.
just a diagnosis of PCOS.   I see lots of moms who are PCOS, but end up with
full, if not oversupplies.   The fact that her breasts did not change during
pregnancy is not a great sign, but I still think you need to wait and see (I think
you said she was still pregnant, right?).

Signs of insulin resistance include:   skin tag growth during pregnancy,
acanthosis nigricans, a big weight gain in the mother during pregnancy,
gestational diabetes (and probe a little on this one--did the woman fail the first test
and then pass the three-hour test?), high cholesterol, and others (can't think
of every single one right now, but look at Lisa Marasco's PCOS
checklist--there should be some others).   I've also had lots of women who have the signs of
insulin resistance, but have normal supply.   I've also had lots of women
with elevated androgens (many signs of which are on the PCOS list) and have a
good supply.   It's most important to see and feel the breasts (in my opinion)
soon after the milk comes in.
