It is a well-known fact that >10%weight loss on day 3 is an indicator that
all may not be well with an infant's feeding - whether it be infant or
maternal reasons.  It is therefore necessary to more closely assess what is
going on in the feeding department, e.g. very sleepy baby, milk supply not
establishing.  At our hospital, we make a closer assessment at this time.
We do not jump into formula supplementation but do make an assessment as to
whether breast changes are occurring and the milk is "coming in".  We will
generally fully supervise all feeds until going well, and will often express
pc and top baby up with EBM in the first instance.  Formula top-ups are only
started if we get no or little response from this and the baby is showing
effects of inadequate intake.  Just an aside, we have a very high induction
and Caesar rate (sadly), indicative of the type of clientele we have - high
maternal request rate.  We do however have nearly 100% commencing BF and
very successful outcomes at 6 weeks (when we finish seeing them at our PNC)
and very few women with > 10% weight loss..
