Texas children's Hospital has a Lactation Support Center, staffed with
IBCLC's.  I would recommend this mom contact them prior to surgery to voice
her concerns.  It is a struggle, sometimes, to get these little ones to eat
after surgery, and dehydration can be quite serious.  That is why our
surgeons are very strict about measuring intake after surgery. We have the
Baby Weigh Scale, but they don't "trust" it.  It is a major frustration for
me!  However, I have found that in situations where the moms are proactive
about  breastfeeding issues - the team is more likely to "loosen up their
reign on measurements", especially with a baby who has proven to tranfer
well at breast.  Typically, these babies are underweight - due to the extra
exertion on the heart, requiring additional calories.  Many of our babies
(those having had cardiac surgery) require augmented breastmilk (24 - 30
calories per ounce).

I had a visit from a mother of a baby that will be undergoing open heart
surgery at the Texas Children's hospital in April.  This baby has been
totally breastfed since birth and at the Dr. visit in preparation for
surgery the mother was told that the baby will have to receive bottles
after the surgery because they had to know the exact amount of milk
taken in after surgery.  This mother has been trying to get her baby to
accept a bottle of expressed milk, but has not had much success.  She
told the nurse that her baby is refusing the bottles but they insist
that there is no other alternative.  This mother lives in the Lafayette
Louisiana area which is about a 4 hour drive to Houston.  Is there
anyone in the Houston area that might be willing to try to intervene for
this mother at the Texas Children's Hospital???
Please let me know so I can refer this mother for further guidance.
