Thank you all for your replies on vasospasm.  Somehow I had forgotten
Jack's B6 recommendation.  I will try that with my current clients.

I had mentioned to a mother warming the nipple well after feeding and
avoiding the cooling of evaporation.  She called today and said that she
kept a soft washcloth on a heating pad nearby and as soon as the baby came
off the breast she put the warm cloth onto her nipple and that helped
greatly.  I will suggest Darillyn's warm diaper idea.  A smooth diaper
would probably feel better than a terry cloth fabric.

The obvious causes of nipple trauma can been resolved but the nipples seem
to "remember" for a while.  As to the claim that vasospasm is not "real" I
wonder where those who make that claim are standing when the nipple turns
white and the mother cries out in pain--pain that can last up to half an hour.
