This won't be anything near a complete response, but I wanted to point
out that babies who refuse formula are far less likely, in my
experience, to replace breastfeeds with solid food... they simply add a
little solid food (and sometimes not enough to satisfy anyone) to the
amount of breastmilk they continue to take in. So many breastfed babies
don't like formula and don't like cereal, but really like natural solid
food ... bananas, sweet potatoes, avocados, etc. I have read the
suggestion of adding olive oil to solid foods, or molasses, where it is
really a question of calories.

I find it helps to suggest to mothers that they breastfeed before *and*
after feeding solid foods. It sometimes provides just a bit more comfort
and helps supply stay up while solid foods are being introduced.
