To try it again - the tube is between the tongue and the breast - the tube
goes along the tongue all the way out a little past the nipple. That way
the baby has complete control of the flow.  It is under the tongue in terms
of the breast being the landing spot - but not in terms of trying to get it
underneath the tongue from the perspective of the baby's palate.

So thinking about a sandwich:

Top palate
breast tissue
tube
tongue
bottom palate

Tube placement on the breast rotates according to the position of the baby.

Does that make more sense?
