I have seen babies react to a fast MER by sliding down the nipple a bit and
clenching their jaws or "biting" to slow the flow. In this case, reducing
moms supply or having her pump with a manual pump or hand express a bit
before nursing may help (in addition to other things that help reduce the
supply).
Also, I see lots of jaw clenchers who leave mom's nipple blanched, white,
and creased. I usually recommend craniosacral therapy for this. You can
often see a white line around their lips from the tension in their facial
muscles when they are latched on and clenching. These babies usually are
doing the "fish lip" look but not always. Depending on how old the baby is,
it may take 4-6 treatments.
Many mothers think CST is crazy but I have seen it work so many miracles.
