I wondered about this too.
Working with women for whom breastfeeding seems doomed is an extremely
difficult experience for us LCs as well and I think sometimes it is hard for even
the best of us to navigate the turbulent dynamics.
I have thought about writing a form letter of sorts meant to be mailed at
the end of these most difficult cases, one that praises and calms in all the
right places. This seems easier to me than writing it at the time. Because of
the fragility of these women, perhaps Lactnet minds with more counseling
expertise/experience might have some ideas??
