Dear Winnie,
       This may not help you this time around, but perhaps next time?  I
always suggest to mom that they *first* contact whichever doctor they feel is more
likely to be cooprative.  (pediatrician or OB/family practice doc)  I tell
the mom that when she has a diagnosis from one doctor, then she can call the
other one and state that "I (or My baby) have/has been diagnosed with
breast/nipple (oral) thrush.  I need medicine for my baby (myself)."  I explain to the
mother that around here *most* of the doctors have finally got it figured out
that if one member of the breastfeeding dyad has thrush, both members need
treatment.  (Still some holdouts, however--sigh)  So since this mom's pediatrician
was unhelpful, perhaps her own OB or family practice doc would recognize the
shooting pains and shiny areolar skin as symptoms of thrush (especially if you
help plant the idea!) and prescribe treatment.  Then, if she's lucky, when she
calls the ped's office back and says she's been diagnosed herself, then maybe
the ped will prescribe for the baby.  Of course, if she has a single family
practice doc who sees both her and the baby, and that doc is uncooperative, it
just may be time to go doctor-shopping.  (Might be time to find a new
pediatrician anyway!)
       Good luck to you and the mom.  Way too many moms wean due to pain of
thrush because doctors are uncooperative.
