What I was trying to say in my post is that it is my understanding that
IBCLCs are supposed to send their reports to physicians. Am I mistaken? If a
mother asks me to do a consult and she has no physicians, or says she does
not want me to report to them, I don't know what else to do. I don't
"insist" that she choose a physician. I explain what I believe to be my
legal/ethical predicament, and respectfully decline to do the consult. I'll
speak to her on the phone and try to give whatever help I can to assure the
safety and comfort of mother and baby. In my vicinity there are lactation
clinics in several hospitals, public health clinics and La Leche League to
refer them to. The homebirth mothers are generally so well educated and so
dedicated that, as a general rule, I am being called in to refer them to the
proper specialist to help them. Almost always they need a tongue clipped,
an antibiotic, an OT, etc.. I am open to any and all ethical/ legal
alternatives.
