Collaboration with Primary Health Care Providers.
This is an example of how I practice as an LC regarding collaborating. I
formally had a private practice and now work as an LC in a hospital but my
practice hasn't changed.
It doesn't matter whether the Mom self-referred or if a PHCP referred her to
me. After an assessment of the problems, Mom and baby, I would ask permission
from Mom to call the PCHP and review the plan of care that she and I had
created. In the hospital I discuss this with the Mom. If I was in private
practice, I would have a form for her to sign before I saw her giving me permission
to discuss her and her baby's care with the appropriate PHCP. I have
enjoyed much respect and appreciation from the PHCPs because I have initiated the
collaboration. It works for me, the Moms and babies that I work with and the
PHCPs. I would not leave a Mom with a plan that I didn't discuss with the PHCP
because I am a consultant and not a PHCP. Many times the PHCP agrees to hold
off on introducing formula for a slow weight gainer because he/she knows that
I will follow this Mom closely and they trust me.
