I'll have a go at answering your questions.
I don't know if donation to a milk bank has been done by a woman who has
induced lactation. Certainly however, women have informally donated milk to
other women who need it.

No, this is not correct. Relactation/induced lactation are a normal backup
mechanisms for situations where a child cannot be breastfed by their mother.
As such, all that is required is a suckling baby (or mechanical or hand
breast expression). Nipple stimulation raises prolactin levels and prolactin
alone can cause development of the milk making structures within the breast.
Some women use hormonal treatments with oestrogen and progesterone but there
is no evidence that it is beneficial. There is a wide variation between
women in how much milk they make when relactating or inducing lactation,
partially a result of individual responsiveness but also due to the
frequency of suckling. The WHO has a great booklet on relactation online
