I think that your reasoning makes perfect sense and illustrates that we need to constantly think about what we are doing and why.  I find that pumping down completely works well whenever a mom is severely engorged.  Once the milk starts flowing,  much better results are usually achieved if mom continues to pump until the breasts are as empty as possible and then starts over from there.  Usually after that, frequent nursing or one-sided nursing is enough to get things under control on a more permanent basis.  But we have all encountered women who are super-producers and are physically unable to comply with the more standard care plans.  This is a care plan which can work for these moms and should be tucked away for future reference.  Thanks for the case study.  I think that this is one of the best ways to learn and is one of the things that we look forward to from our participation in Lactnet
