I have a mom who is going home in a couple of hours. She had mastitis with
her first two children and has been using a nipple shield with this baby--
per her sister's pediatrician--to prevent mastitis (and sore nipples).
This mastitis prevention seems contrary to everything that I know about
shields, but supposedly the ped said it would help drain the ducts better
because of even breast coverage. The baby latches fine without the shield.

Mom didn't notice an overabundance of milk with her first baby. She
just "passed out" one day from being weak with mastitis. She was feeding
30/30. The second baby only nursed 5/5. Mom did not pump afterwards. She
got mastitis with both babies at about 3 weeks postpartum.

What do you recommend that I tell her regarding this baby? What tips might
help her prevent another round of mastitis? Should she pump after the baby
nurses? I would appreciate any quick tips.

Thank you,
Joyce Coleman, RN, IBCLC
