I do not routinely ask mother to pump if she is using a nipple shield.
I watch milk transfer, voids and stools and weight gain. There are a
variety of reasons, the most frequent being that the mother is usually
very stressed by pain and likely to quit breastfeeding if the pain
persists. Adding routine pumping adds to her stress. So I don't advise
pumping until I see how well the shield is working and if the baby is
thriving. I do encourage mother to not overuse the shield over a
lengthy period of time because she may have difficulty maintaining an
abundant milk supply.
