Here is what I used for my babysitter when I had to go back to work.

It is most important to me to feed my daughter, Olivia, with breastmilk and
not formula.  This takes your cooperation also as I will be coming home on
my lunch hour to nurse her.  Please do not feed her the bottle any time near
12 or 5 so she will be able to eat then.  If she is starving, you might give
her an ounce or 2 to hold her over till I get home.  There will be bottles
of breastmilk in the refridgerator.  If she runs out, there are some frozen
breastmilk "cubes" in the freezer.  You can thaw that out one or two cubes
at a time in the bottle.  Run the bottle under hot water or put it in a cup
of hot water to thaw it out.  Do not use the microwave to thaw it out as
this kills some of the antibiotic properties of breastmilk.  Breast milk
looks different then formula.  Sometimes the fats in it seperate.  Just
swirl it around a bit and it is fine.  It also looks more watery then
formula.  Don't worry about it.  She will probably eat about 3 or 4 oz.
Every  3 hours at least.  You will soon learn her signals for being hungry.
She is a pretty easy burper but sometimes when she doesn't burp, if you lay
her down for a few minutes, she will after that.
