I have had several mothers make sufficient milk after breast reduction
surgery. One that comes to mind had surgery when she was 20 and had her first baby
in her 30's. The baby was term but was in NICU for about a week. During this
time mom did a combination of pumping and breastfeeding. She had periareolar
incisions and the typical anchor incisions under her breasts. She had no idea
exactly what the surgeon had done, but her milk came in in a timely fashion and
she went on to have oversupply. She is now successfully breastfeeding her
second baby. In the cases where I've seen adequate supplies after reduction surgery
we didn't do anything in particular except closely follow mother and baby and
inform her of the risk for reduced supply and the need for frequent wt.
checks. In my experience sometimes babies are satisfied short term but wt. and
supply falter after a few weeks so these mothers need close follow up. I've also
noted that some mothers do not tell their physician or the nurses caring for
them that they have had surgery and the cosmetic results can be so good that it
is not evident to the untrained eye. They then come to our attention when
things are not going well after discharge.
