Once, years ago, I tried an SNS on such a woman.  Not for any good reason,
just desperation, a non-latcher, and a low supply.  By golly, that little
stickie outie tip on the tubing told both mother and baby exactly where the
bull's eye was, and they did well with it.  (This was a baby *I* had been
able to latch, but not the mom, so the only latch problem they were having
was technical; no edema.)  I'd run it from the bottom not the top, if I were
doing it again.

As to a hands-free pumping bra: there are dirt cheap versions, one being a
zip-up-the-front (or any other) sports bra.  The fabric allows mom to cut
holes that don't fray.  She cuts a circle just large enough to slip the
flange through, and the stretchy fabric holds it in place.
