I am one of 3 hospital based LC's at a hospital in southeastern USA.  We
total 1.8 FTE's.  Until now we have not charged for our services but we are
working toward that goal.  We know we need an assessment sheet for
documentation that is different from what the floor nurses use for their
documentation.  We also need a documentation tool to use for outpatients.
I have collected some from workshops over the years.  I know there is a
wealth of information on LACTNET and I see no need to reinvent the wheel.
Anyone with any information they are willing to share, please e-mail me
privately and I will send you my address.
Also I would appreciate any information on what is required for
reimburesment.
