'm finding the discussion on nipple shield and flange sizes very
interesting. Old LC's can still learn something new!!  I plan to see if I
can find one of the circle tools mentioned in our office supply catalogue.
I have alwasy just eye-balled nipple size, but in our outpatient store we
let moms try out the different sizes. and that really helps.   I have found
that my eye-balling is frequently wrong, as nipple size changes when pulled
into a pump..  I usually let mom decide which size is more comfortable for
her.  I have also had many moms find that they can express much more milk in
a shorter time frame with a larger flange.  One mom I remember thought that
she was losing her milk as she hadn't been able to get much when pumping.
When we tried a larger flange she was able to pump 2 ozs. in a very short
time and she was estatic.
As far as nipple shields.  We only carry the newborn size.  They do use the
small size in our NICU, but usually for babies under 5lbs.  The general
feeling is that by 5lbs most babies mouths are big enough for the newborn
shield or  ideally they should be off the shield completely.  I didn't even
know that Medela made an "extra-small" .  I bet the Target people ordered
the wrong size by mistake because they didn't know anything about what they
were ordering.
