We also use all the bendy bumpers and such, and we are also supposed to
swaddle with hands near the face, but I see so many who wrap the hands in
the edges of the blanket or down around the tummy. Old habits are hard to
break I know. We use a very very soft silastic tube called a nutricath -
they are not at all like any other feeding tube I have ever seen. The
majority of babies do not complain and most barely react when this tube is
placed back. Yes we tape the down very well too, but when you have limited
space to tape - for instance in the case of a kid with a nasal cannula, that
sometimes can be difficult. I still think that baby should be allowed to
have his hands to mouth on and comfort himself with. 

As far as the rules going out the window with preemies you are absolutely
right about that!
