I've also seen one woman who had a full milk supply for her second baby
after reduction surgery.  Her surgery was classified as a mastopexy
(breast lift), but the surgeon did remove 450 and 500 grams of tissue
from each breast, respectively.  She fed her first baby with a feeding
tube and did some additional pumping, then went to breast every few
hours, and bottles in between.  With the second baby, she pumped often
and gave him several  bottles of ebm for the first month or so out of
insecurity and desire to space the feedings a little more.  She is now
exclusively breastfeeding, and the baby is gaining well.

I have only had one mom who had a true reduction mammaplasty have close
to a full milk supply.  She only has to give 4 oz of abm a day.  We used
a syringe and tube at breast, switched to a lact-aid when baby got a
little stronger, and she has taken domperidone and goat's rue.  She was
able to stop using the lact-aid and supplementing when the baby started
taking significant amounts of solids.  She worked very, very hard up
until about 4 months to keep the milk flowing.
