As I sit here trying to gear up for providing lactation consults in a Pedi.
clinic I am reading all I can, including Priscilla Bornmann's legal paper in
the LLL series.  I am not finding one thing that I think I'll need, and that
is a form or template for sending home instructions with patients.
Presently I find myself writing down lists of things for the mother as she
leaves on  a piece of notebook paper, which, obviously is inadequate and
unprofessional.  Of course at this point I do progress notes, a letter to
the physician or nurse midwife that referred the pt., but don't have
anything too formal or fancy for take-home instructions.

ANyone care to share?  (I'd be especially thrilled if it were in English and
Spanish......)
