I used one on a nearly 34 week preemie today.  He could not maintain negative
pressure to hold his latch and transfer milk.  But with the small nipple
shield (it was perfect for moms' nipples and the tiny little 33 week mouth)
it was like magic.  Transfer with each suckle!  He fell off of the breast
milk drunk.  I love it when these little ones do so well.  I have had luck
with some who can sustain the negative pressure without a shield but that
shield can get breastfeeding going in many cases.  And it gets the darn
bottle out of the baby's mouth when mom is there for feedings in our NICU!
This tiny little guy was being forcefed every 4 hours 3 ounces of moms milk
(and left to wait when he is hungry) and he was just 5 days old!  I gasped
when the mom told me this!  I don't get this lack of common sense about
stomach capacity, digestion of breastmilk, etc.!  These moms are told many
times to feed every 4 hours at home on discharge!  I have talked until I am
blue in the face.  We try to educate as many of the moms as we can.  This mom
knows.  She will call us, but how many others won't?  Too many.  The same
with all of the bad advice they get from their Doctors.  Today one NICU mom
told me she stopped pumping her milk because of a cortisone cream the OB Dr.
has her putting on her incision.  I said, "what?!"

Another day of pulling out my hair.
