Yes, I've had days like that! (And I fear one like that
tomorrow-it's my first day back after a 6 week medical leave and I
am praying for a light day to get back in the groove) It also seems
that the more you have to see, the higher the percentage (not just
the numbers) of moms having problems! Do you get out in less time
when (or if) the census is lower? If so, this would certainly be an
argument you can use. Do you keep any kind of a time log as to how
much time you spend with? We do this regularly. We have a
checksheet and mark for each contact as <15 min, 16-30 min, 31-45,
46-1 hour and > 1 hour for inpatients. Phone calls are <5 min, 6-10
min, 11-20 min and > 20 min. Outpatient visits are in 1/2 hour
increments up to 90 min and then > 90 min. For all of these, the
time spend in finding information for the mom and time in charting
on the mom/baby is part of the time spent with her. In addition, we
indicate time spent in staff education, recordkeeping, etc. This
can help you justify your time spent as well as helping you see
where the time goes.
Are you doing a lot of general breastfeeding teaching or only
helping with concerns and problems. If it is teaching, this would
be a great argument for doing more staff education so you can trust
that staff is giving consistent, correct information, leaving you
more time for working with the problems.
