I saw a mother last week whose third baby had a very tight lingual frenulum.
He weighed over 11 lb at birth.  She was discouraged from pursuing a
frenotomy by the pediatrician who was doing rounds the day her baby was
checked, but we kept the idea warm and when she came back for a PKU and
weight check on day 5, he was 6 oz. OVER birthweight but she was very sore.
She still wanted time to think it over, as she had breastfed her two others
without problems and was hesitant to do anything drastic to this baby.  A
few days of no improvement in her soreness, despite careful attention to
very asymmetrical latch, convinced her to go for the frenotomy and it was
arranged a day later.  (Normally there is no wait at all but summer is upon
us and ENTs go on holiday like everyone else.)
Relief was immediate, as she fed him painlessly right after the procedure.
She reported the baby did not react in any way to the clipping but protested
at having the result checked afterwards - didn't want any more fingers in
his mouth!
