If hospitals / midwives at home are working towards the Baby Friendly
guidelines then every woman whose birth they attend, and for whom they care
postnatally should be taught how to hand express.  I know that there is a
huge working/pumping culture in the USA (and in some places here in the UK),
but for the kind of problem hirings hand expression may even help to 'head
the problem off at the pass'.  At the risk of sounding naive, don't state,
local, regional, whatever protocols include teaching every mother to hand
express during the first days after she had had her baby?  It doesn't happen
everywhere here, but, due to BFI it actually is recognised (often grudingly)
that it is actually good practice.

Here in the UK a lot of pump hire is done by the voluntary organisations
(three of the four have pump hire schemes) -- which arguably undercuts  the
BFI and hand expressing and also the imperative for government to do
something about it through our National Health Service.
