It is not desirable for IBCLCs to participate in "under the counter" advise
about medications.  Medications are the purvue of the physician.  The IBCLC
may inform physicians that domperidone is legally available in the US from
compounding pharmacies (every community has one).  A good article to review
would be:

O da Silva, et al:  Effect of domperidone on milk production in mothers of
premature newborns:  a randomized, double-blind, placebo-controlled trial.
CMAJ 2001, 164(1):17-21.

It would be great to have a comparative study of efficacy, but I'm not
personally aware of anything looking at domperidone compared to
metaclopramide.
