Dear Wise Ones Worldwide~
I am working with a woman who presents a baffling case. She has given me
permission to share her story with you.
My initial contact with her occurred about 2 weeks ago. Her pediatrician
asked me to see her for two reasons--her baby was being treated for Thrush,
but the mom continued to complain of sore nipples so he wanted the latch to
be evaluated, and he felt that she needed some reasurrance that her son
was "doing well". ( he has apparently been seen for problems with a "stuffy
nose" which she felt made breastfeeding difficult) He wanted her to see a
lactation consultant prior to seeing her own physician.
I examined her breasts and nipples--no "obvious" signs of yeast, no
cracking, blisters, scabs, redness or blanching. The baby latched on
beautifully and got 2 ounces in a sort of "snack" nursing. The mom did not
express feeling any pain. He was smiling and content and gaining weight. I
did not see any more evidence of thrush in his mouth, and he had only the
barest "suggestion" of a diaper rash.
As you can read in her letter below, the mom has a significant history of
yeast infections. She is very aware of treatments and management. The only
additional things that I came up with for her to try were Pat Gima's yeast
treatment and a prescription for Newman's APNO. If I remember correctly,
that day she was given a loading dose of Diflucan 300mg and then two weeks
worth at 150mg a day. That would have been her second round of it.
Yesterday, this mom spoke with my colleague and seems to be getting very
weary of the whole situation. She talks about giving up nursing, but then
says that she wants to continue. She is planning to get second opinions
from other physicians, and is also consulting with healthcare providers in
Spain, which is her homeland.
What also concerns me is that she reports that she is losing weight and
frequently expressed concerns and worries about her son's health both to me
and to my colleague. We are both beginning to wonder if there is more to
this than her complaints of persistant yeast infections. Could she be
experiencing postpartum depression? Could there be another physical ailment
or breast condition that I missed?
Any thoughts or ideas are welcome and very much appreciated.
Her letter explaining her history in her own words is below.

thank you~
Kirsten RN IBCLC

Thanks so much for your concerns. Here you have a brief story of myself.

I've got a daughter, Anna who is 4.5 year old. I nursed her for a year.
Right after that I had my first vagina yeast infection. I treated it and
went away till the next month. Since then I've been fighting Candida. I went
to different doctors and dieticians. Wheat and dairy intolerance were found.
I've been in a yeast , dairy, wheat and sugar free since then almost. A year
and a half ago, they found I'm hypoglycemic, so no sugar again, because it
really made candida grow. Things were better with this diet and a
homeopathic treatment for my weak immune system. So I became pregnant a year
ago. My baby boy, Ruben, was born on 11/11/02. During the whole pregnancy I
got vagina yeast infections every time my husband and I made love. So my OB
recommend condom protection.
Ruben will be 3 months next Tuesday and since a month ago or more we've been
having this problem. We treated it with Diflucan for 2 weeks and nothing
happen.
Now we are with Diflucan for 2 more weeks till Sunday, and I'm also taking 4
kyolic garlic capsules, 4 Echinacea pills and 6 bifidus capsules
(PRimadophilus) every day along with my diet. I clean my nipples and the
baby diaper rush with water and white vinegar every time I nurse him. I put
Newman's special nipple ointment on my breast and a fungicide cream to
Ruben's rush.
Thanks again for your help.
