Dear Lactnetters,
Help!  But first I need to vent a tad.  This weekend I was told by the SCN
nurse to give some hydrogel dressings and a shield to one of the moms there.
Seems the Pediatrician told the mom she needed to get these things "just in
case" she got sore nipples.  The mom told me that the Ped told her to wear
the shield in the shower. (?????)  Whatever.  The next day I asked the Ped
what he meant by the shower thing and he responded oh, that's not what I
meant, but I do want her to have these things just in case.  I had to put my
two cents in and tell him that shields weren't meant for sore nipples and
had some risks.  He came back with... oh, are you the one who tells moms to
shake her breasts?  Just some background.  This is a Ped who has given an
order that Lactation is not to see "his" patient's moms unless he gives
specific permission.  Regardless of what the mom wants or what the OB
orders.

Anyway, I told him that if he meant massage, stroke, and shake to help get
the let-down going, yes.  So he wants to know the medical rationale for
that.  Because it works?  Not good enough.  Where is the scientific data?  I
was in such shock that I couldn't come up with anything.  I guess I should
have seen it coming, but I didn't.

So the question:  do any of you know where the "shake" in massage, stroke,
and shake comes from?  I know that it is on the Medela handout for
engorgement and referenced from the Marmet technique.  And I plan to call
the phone # on the handout to ask, but in the meantime anyone else have any
ideas?

After he walked away, I realized I should have asked him what his concern
was about the technique, but he'd left the building by then.

Anyway, I'd appreciate any help.
