Dear Susan,
One form is all you need.  It is understood in the HIPAA regulations that
clinicians involved with your care as a patient can share information both in
writing and verbal consults.
The HIPAA regulations are trying to cut down on people who are not involved
with a patient's care who get access to their records.  There is also a push
to increase privacy.
