The only time I've seen bloody clots like this was with mastitis.  As I
understand it, the blobs were a result of the coagulating effect that Staph
aureus can have; the blood *with* the clots was from irritation or maybe
from the passage of the clot through a narrow nipple pore.  Culturing her
milk might identify the organism.  For my client, systemic antibiotics
cleared it right up.

Another client who had clots only (no blood), had them resolve as soon as
she switched pump flanges.  Has your client perhaps switched to a new style
pump or flange recently?
