We have a bunch of photos in the Breastfeeding Atlas of twins nursing in
various positions, including lying down.  It is somewhat difficult to
verbally describe positioning, but I'll take a stab at this.  The shape of
the mother's breasts impacts her ability to nurse lying down.  The mother in
the Atlas has rather widely spaced breasts that are somewhat "long",
allowing her to have a baby on each side.  She is using a body pillow shaped
in a horse-shoe configuration around her and the babies to hold and support
them all together as she dozes.

I have an interest in the Atlas as I am the co-author.
