We have discussed a few cases before, might get some ideas in archives. I
would not advise expressing/pumping, but instead would suggest cold,
especially underarms and maybe even cabbage if she's really uncomfortable. I
usually suggest lying supine to 'elevate' the breasts to decrease the edema
(in other words not keeping the breasts dependent); not sure if she can do
this comfortably, safely at this stage of pregnancy. Maybe just for short
periods. Might this episode of engorgement and leaking be a sign of
impending (preterm) labor? Might this indicate some kind of placental
separation (knowing the hormonal changes that precipitate lactogenesis)?
Even a very small tear? Is she at risk for any of this? Just some random
thoughts.
