Kathy, I too, have had this experience only once.  Mom was proficient nurser,
this was 3rd child.  We worked together for some time with this.  Baby would
vomit most of feedings if taken from breast but not from bottle.  Eventually,
this child ended up failure to thrive and after much--translated months--of
extensive workups it was determined she had extremely bad reflux.  Never
really nailed down a reason for the vomiting at the breast...So anecdotly-yes
I have seen it, but no idea what caused it.
