A few things come to mind:  This may be the classic "high-need" child (won't
take "mother-substitutes," like bottle/pacifier, "inflexible temperment,"
etc.--- see William Sears' book "The Fussy Baby") ; could be some sensory
defensiveness, or could be overactive milk ejection, or a combination of the
latter two, in other words, the baby "copes" best with the flow when it is
more regulated and not so fluctuating.

Also, excuse me for asking the obvious, but could the history of ear
infections be relevant?
