Ashleigh, these breasts may be engorged. But not with milk. With fluid.
Did she have lots of IV fluids? Did she have trouble with blood
presssure before or during labor? How much Pitocin did she get? Continue
pumping, but try reverse pressure softening (see archives). It works.
Tell her to remain patient. It will come. She may not take the freeway
to get to her goal, but she can get there by the side roads. Cheryl

Subject: difficult latch

I have a patient that is a first time mom having difficulty with
latching. Baby has anexcellent rooting reflex, but uncoordinated suck.
We are now inot day two and the baby is still unable to latch. Mom's
breast are extremely firm and taut, nipples flat. They almost feel like
engorged breasts. She has no history of implants. She states that she
did feel changes in her breasts during her pregnancy. We used a
inverted syringe yesterday to help pull the nipples out withiut success.
Have tried nursing/pumping every 3 hours. Last pumping obtained 2cc.
Tried "nipple sandwich" without success. Any ides before hse completely
gives up. She has great family support, but is very frustrated. NIght
shift began supplementing with 15cc formula after feeds last night and
baby starting to look slightly jaundiced. Thank you in advance for your
help. Ashleigh Beyer, RN, CCE, awaiting IBCLC results
