I cannot personally recall a failure to resolve with this many multiple
treatments of yeast although resistance to GV alone does occur.   I would
culture the nipples and a mid-stream catch of milk.   I am finding that I
must use two swabs one to culture for yeast and the other to culture for
bacteria or it takes a month to get the results.  I have had one mom that
needed to be on diflucan 200mg per day for over 6 weeks.  I kept her on the
diflucan (her and the infant) until her symptoms completely resolved for at
least 7 days.
Is the baby being treated?  Is she sterilizing everything that comes in
contact with her milk, breasts or the babies mouth?  Is she limiting sugar
and  foods with yeast?

Does she have underlying disease,  ie. is she possibly diabetic?  (The mom
above was on steroids for asthma)

You might want to do nothing on the nipples for a few days and let it declare
itself  (infection vs dermatitis) and if it doesn't, then at least you'll
have some culture results and maybe an answer.
