Let me add one more level to this complicated case.
Is it possible that this was an unplanned pregnancy and
there is still some ambivalence about the baby.
Breast implants between babies seems strange to me,
especially for a mom who bf her first two, even briefly.
Who at home understands/is supportive of bf? Can the father
and grandmother be educated so that they can offer bf
friendly support? I would assume that their intentions are
good, but they may have no idea how to go about being
supportive.
I wouldn't assume that her storage capacity has diminished
because she doesn't remember nursing the older kids so often.
Memory is often not accurate 6 or 8 years later, she didn't
nurse them for a long time. Perhaps her supply diminished then
because they weren't getting enough. There are all kinds of
possibilities.
If there has been an increase in leaking since she passed the
clots, and that was only in the last couple of days, then
it seems that that may have been at least part of the problem.
Does the clotting need to be checked out further? Seems to me
to be within range of normal, but this is not my area of
expertise.
I imagine you will follow up. Keep us posted.
