I need assistance finding resources for advising a breastfeeding mother.
She has an unusual condition. The diagnosis is Cryptogenic Chronic Active
Hepatitis with recent leanings towards Fatty Liver (Non-Alcoholic
Steatohepatitis).
A brief (as possible) history is: She was diagnosed with this condition a
few years ago. She had an liver biopsy with inconclusive results. All forms
of viral causes were ruled out. She had one elevated cortisol level.
(normal range is 1-6.5 hers was 7.5) She had subsequent normal liver
function tests and decided to have a baby. The baby is about 20 weeks old
now. She has been exclusively breastfeeding and does not want to stop. Baby
has been gaining and shows no outward signs of abnormal development.
Recent liver funtion tests returned elevated to the range of 289AST and 289
ALT. This was a dramatic increase since her last lab work when the baby was
7 weeks old (13 weeks ago)
My Questions are:
1. Is her breastmilk comprimised due to the fact that good liver function
filters out contaminants?
2. Could this condition be aggravated by pregnancy and/or lactation?
3. Where can she have her milk evaluated for contaminants?
4. Should (and where) could the baby be evaluated for any contaminants?
She knows much more about her disease than I do, so please any explanation
of how to go forward in counseling her would be appreciated.
